If so, your child will need to attend a free screening appointment in order to be eligible for a Title 1 spot. The appointment will take about 30 minutes, depending on the child's cooperation. A parent/guardian must attend the appointment with the child, and must bring TWO forms of proof of residency (printed documents with parent/guardian name & address: utility/phone/gas bill, driver's license, insurance card, bank statement, etc.).
